Choosing a health insurance plan is a once-a-year decision most people make in twenty minutes, usually by picking the middle premium. That shortcut ignores the two numbers that actually decide what a plan costs you — the total yearly spend and the worst-case spend — and it ignores the network list that decides whether your doctors are covered at all. Here is the framework that gets it right.
A premium is a certain cost; a deductible is a possible one. The honest comparison multiplies the premium by 12, adds the care you realistically expect, and then separately checks the disaster case: premium times 12 plus the out-of-pocket maximum. A plan that wins on the likely year and loses catastrophically on the bad year is not the cheaper plan.
